How Do I Know If My Doctor is Part of a Patient-Centered Medical Home?

A patient-centered medical home, or PCMH for short, describes a model of care developed to make your experience at the doctor’s office more streamlined and comprehensive, resulting in better care and reduced costs. Within the medical home model, PCMH doctors are certified in leading care teams to focus on patients’ unique needs. PCMH-designated practices outperform non-designated practices across the board in many categories: • 16% lower rate of ER visits for adults • 20% lower rate of primary care sensitive ER visits for adults • 27% lower rate of ambulatory care sensitive inpatient stays for adults • 18% lower rate of pediatric ER visits • 26% lower rate of primary care sensitive pediatric ER visits Blue Cross’ program leads the nation with 4,630 physicians in 1,700 practices. Blue Cross PCMH-designated practices are in 80 of Michigan’s 83 counties. How can you find a PCMH doctor or know if your primary physician is a PCMH doctor? There are two ways, either through the BCBSM Mobile App or online at the BCBSM website. All PCMH physicians are distinguished by a small blue PCMH oval found in their description. FIND A PCMH DOCTOR BY WEB:
  • Log in at BCBSM.com
  • Click on Doctors + Hospitals
  • Click on Find a Doctor
  • Click on Find [or View] a PCP
  • If you clicked ‘Find,’ based on your location, a list of nearby doctors will appear. On the left side of the screen, scroll down to the “Quality” section to select the PCMH filter. After selected, all the physicians near you that work in a PCMH-designated practice will appear on the right side of the screen.
  • If you clicked ‘View,’ click on the physician’s name to see if their profile has a small blue PCMH oval in the description.
FIND A PCMH DOCTOR BY BCBSM MOBILE APP:
  • Log in on the BCBSM Mobile App
  • Click on Find a Doctor
  • Click on Primary Care Physician
  • Click on Find [or View] a PCP
  • If you clicked ‘Find,’ based on your location, a list of nearby doctors will appear. Click on “Filter” at the top of the page. Scroll down to the “Quality” section and select the PCMH option. After selected, all the physicians near you that work in a PCMH-designated practice will appear on the screen.
  • If you clicked ‘View,’ click on the physician’s name to see if their profile has a small blue PCMH oval in the description.
